Here is what a question looks like

Airway, Respiration and Ventilation

A 62-year-old male is found unresponsive after a fall. He has snoring respirations and blood in the oropharynx. After suctioning and opening the airway with a jaw-thrust, you are unable to ventilate with a bag-valve mask despite repositioning. What is the most appropriate next intervention?

  • A. Insert a nasopharyngeal airway and continue ventilation attempts
  • B. Perform immediate needle cricothyrotomy
  • C. Insert a supraglottic airway device
  • D. Begin chest compressions while preparing for intubation
Show the answer
C. Insert a supraglottic airway device

When basic airway maneuvers and bag-mask ventilation fail in an unresponsive patient, a supraglottic airway is the appropriate next step. Needle cricothyrotomy is reserved for failed advanced airway attempts. NPA is relatively contraindicated with significant oral/facial bleeding, and compressions are not indicated without cardiac arrest.
